Message to Healthcare Providers
Effective Feb 24 – DCO will no longer confirm receipt of faxed referrals. Please check your fax transmission reports to verify successful submissions or contact our office for assistance. We appreciate your support.
What to Expect When Faxing – DCO forwards your faxed referral to one of the six community diabetes programs offering services in Ottawa and Kemptville. The diabetes program contacts your patient to book the appointment (s) and reports to your office by fax with a status update. Wait times can vary across programs, please contact DCO if you have any questions.
eReferral to DCO – Ocean eReferral is available to Ontario health service providers free of charge, as part of the Ministry of Health funded Ontario eServices Program.
The six partner Diabetes Education Programs (DEP) are:
- Bruyère Academic Family Health Team (FHT) DEP
- Community Diabetes Education Program of Ottawa
- Eastern Ottawa Community FHT DEP
- Ottawa Hospital Academic FHT Healthy Lifestyle DEP
- Wabano Centre for Aboriginal Health Diabetes Program DEP
- Kemptville District Hospital and Ottawa South DEP; offering virtual services to Ottawa South Residents
